Preparing for Adulthood


Whether it is into college, community provision or a supported work placement, moving on from school is a worrying time for learners and their families / carers.


We work closely with other agencies to make the move into a new setting as seamless as possible e.g. Social Care, Preparing for Adulthood advisory team, Behaviour Assessment Team, Learning Disability Nurses and our own Family Worker team. Transitions into new settings are carefully planned and may take place across an extended period throughout the final year at school.


We share as much information as possible before a student leaves, so that student needs are met and understood by those who will support them in the future. We are involved in supporting the conversion of a student’s Statement of SEN to an Education Health Care Plan, and will support parents / carers with this.
